Health Statement/Immunizations All children must have a current health form on file signed by a physician, stating that the child has been examined within the past year and is physically able to participate in our program. Each child enrolled must meet applicable immunization requirements specified by the Texas Department of Health. treatment or immunization conflict with the tenets and practices of a recognized religious organization of which the parent is an adherent member or if the parent has filed a completed, notarized affidavit citing conscientious objection to vaccinations.
Vision and Hearing ScreeningAll children four years and older should be screened for possible vision and hearing problems. When possible, the School will provide vision and hearing screens through the UTHSC nursing school or private individuals.
AllergiesIf your child has any significant allergies, please notify the Teacher and the Office in writing. Please include any appropriate action to be taken, medication to be administered and whom to notify.
MedicationsPlease do not send medications in your child’s backpack or lunch. All medications must be brought to the office where they will be stored in a locked cabinet. Since it is impossible to keep refrigerated medications in a place inaccessible to children, we ask that you not send any medication requiring refrigeration. All medications should be in the original container. In order to administer medication, a parent must sign a Medication Form stating dosage and times and are only administered by Office Staff or the child’s Teacher. Date, time, dosage and name of the Staff who administered the medication are noted on the form.
IllnessIf your child becomes ill at school, he/she will be comfortably isolated in the office and you will be contacted. Please do not send your child back to school until he/she is free of symptoms (fever, diarrhea, vomiting, etc.) for 24 hours. In case of a highly contagious illness in your child’s class, you will be notified. Staff is required to send home any child with the above symptoms.
